Submitting is voluntary, does not guarantee that you will be contacted about, qualify for, or be enrolled in any study, and is not a substitute for the informed consent process. DCR may share your information with its affiliates, research personnel, service providers, study sponsors, institutional review boards, ethics committees, regulatory authorities, and others as permitted or required by law for recruitment, screening, compliance, and related administrative purposes. DCR uses reasonable safeguards to protect your information, but no online form, database, or electronic transmission is completely secure. You may request removal from DCR’s recruitment database or withdraw your consent for future research recruitment communications at any time by emailing DCRHIPAA@davita.com; DCR may retain certain information as required or permitted by law, including for compliance, recordkeeping, and administrative purposes.
